Albertans living with chronic conditions benefit from home monitoring

A new home monitoring program aimed at supporting better management of chronic health conditions through digital technology is already benefitting patients and their health providers in central Alberta – and serving as national model for remote monitoring technology.

The Alberta Central Zone Home Health Monitoring (HHM) Project is a unique collaboration between Boehringer Ingelheim Canada, Alberta Central Zone Primary Care Networks, Health Cities Edmonton, Telus Health, Alberta Innovates and the Alberta Government’s Ministry of Seniors and Housing. The project, which continues to scale, provides a community-based home health monitoring digital solution for people living with chronic conditions.

The innovative technology provides a new way to deliver chronic disease management programming virtually, while alleviating stress on the health system. Patients receive a regular touch point with a healthcare professional, giving them the opportunity to ask questions and address concerns about their health. Results of the project to date have shown high satisfaction among patients, who report a greater sense of being cared for and greater confidence in their ability to self-manage their condition; as well as satisfaction from receiving care closer to home, which is especially significant when considering rural healthcare challenges. 

Healthcare providers have also reported satisfaction with home health monitoring, which has led to decreased acute care visits and hospital admissions.
